How do I report a claim?
To report a claim, call (800) 987-3373. The claim center operates 24 hours a day. A customer service representative will take all the necessary information and assign a claim number. The insured will also be given the address and phone number of the claim office. The customer service representative cannot handle the claim nor give any advice. Claim Counsel will be assigned the next business day.
Claims may also be submitted online at http://webclaims.zurichna.com. After submitting the claim a Web Incident ID will be assigned. The person reporting the loss will receive a return e-mail. Once the claim has been received by the Care Center and assigned, another e-mail will be sent to the reporter. A complete confirmation packet will be mailed to the insured.

How do I place a piece of business?
Print application and appropriate supplemental applications from web site and fax completed, signed and dated paper application to Zurich Insurance Brokerage Services.

What is needed to obtain a quote?
To receive a quote, we need the following information:
- Completed and signed brokerage agreement with Zurich Insurance Brokerage Services
- Completed Application(s)
- CLIA certificate
- Two years financial statements (tax returns)
- Five years hard copy loss runs
- Resumes of key personnel and licenses
- Standard operating procedure manual/safety program
- Brochures
- Spill plan/SPCC/emergency plan
- Sample contract for use with clients
